When Boston trolley operator Aiden Quinn ran into a trolley ahead of him last May, he admitted he was texting his girlfriend.  Today the 24-year-old was arraigned and could spend three years in prison for gross negligence operating a train.

OSHA is opening a probe into what killed a monorail operator at Walt Disney World in Florida. 21-year-old Austin Wuennenberg’s died Sunday when a monorail crashed into a second one around 2.a.m. It is the first death ever in the history of the monorail.

The subway cars involved in a deadly crash in the nation’s capital Monday were cited as unsafe three years ago, but were never replaced because of the cost.  That information is emerging as 19 NTSB investigators search for clues into the cause of the rush hour wreck that killed nine people and injured 70.

When procedures are properly followed, unaccompanied minors arrive at their chosen destination. But that wasn't the case last weekend when two unaccompanied girls were placed on wrong Continental Flights within two days.

Continental Flight 61 was en route from Brussels, Belgium, to Newark, New Jersey, when the pilot died mid-flight today. Passengers were unaware of the event, since the co-pilots landed safely, until they saw the emergency vehicles at the end of the runway.

Investigators are facing challenges in recovering the black box data recorders, wreckage, and bodies from an Air France plane that suddenly plunged into the Atlantic Ocean June 1 with 228 on board.  Still, they believe they're getting closer to determining the cause of the crash.

Today the National Transportation Safety Board (NTSB) will begin three days of hearings into the increasing likelihood of collisions with birds, which has doubled at 13 major airports over the last decade.  Listen to the hearings live.

The downing of Air France flight 447 is still a mystery and will remain so until the black box recorders are located. The U.S. Navy is helping with recovery using devices that can detect sounds as low as 20,000 feet, but the ocean in the area off the Brazil coast may be even deeper. 17 bodies have been recovered so far.

Remains of an airplane, metallic material, oil and an airplane seat,  have been spotted in the sea near where Air France Flight 447 is believed to have disappeared Sunday night on a flight from Rio de Janeiro to Paris. Brazil’s Air Force reports there is very little material given the size of the plane with 228 people on board.

What happened to Air France Flight 447? There was no emergency signal after it took off from Brazil last night on an overnight flight to Paris when it just dropped off radar three hours into the flight.   The airline believes there is no hope, though a search operation is underway.

The Federal Aviation Administration is releasing previously withheld data on where and when planes have struck birds and other animals over the last 19 years. JFK International tops the list with 30 accidents among the 89,000 reported nationwide.

You’ve seen the warning lights in the airplane bathroom during turbulence,“Return to your Seat".  A Texas woman was thrown into the ceiling and broke her neck during a Saturday flight over Houston. She suffered the same sort of break as the late actor Christopher Reeve and is undergoing surgery to try and correct the paralysis.

United Airlines passengers who are wider than the arm rests, will have to purchase another seat or wait until a flight with open seats. It's a way to both capture additional revenue and to answer hundreds of comsumer complaints the airline says it has received about wide-bodied flyers.

A passenger aboard a King Air twin-engine propeller plane took the controls and landed successfully at Southwest Florida International Airport when the pilot died mid-flight on Sunday.

Without a cockpit recorder we may never know what caused a small plane to crash just short of a Butte, Montana runway Sunday afternoon. The plane may have been overloaded and the FAA says it was not certified to carry commercial passengers.
