The National Transportation Safety Board has found failures in train control systems audio frequency track circuits that allowed one train to plow into the back of another in Washington D.C. last June. Other areas around the country use the same technology, but no one has an accounting of how many.

The first wrongful death lawsuit has been filed in connection with last Friday's collision of a Metrolink commuter train with a freight train. Southern California has been slow to adopt new technology that serves to prevent human error. With the technology, "there's no question the accident would not have occured," says attorney Paul Kiesel.
